Introducing MyMedicare
As part of an ongoing commitment to strengthening Medicare for all Australians, the Australian Government has introduced MyMedicare, a voluntary patient registration model that aims to formalise the relationship between patients, their general practice, general practitioner (GP) and primary care teams. Seeing your GP regularly and formalising the relationship you have with your GP and practice through MyMedicare can lead to better health outcomes.

What are the benefits of MyMedicare?
- Greater continuity of care with your family doctor.
- Longer MBS telephone calls with your usual general practice, allowing for more comprehensive discussions about your health with your doctor.
- A dedicated preventative health team commited to helping ensure you receive the long-term health benefits you deserve.
- Formalised relationships with your practice and your GP.
Register with a MyMedicare registration form
You are able to register for MyMedicare by completing a registration form in one of the following ways:
- You can ask one of our receptionists for a hard copy of the registration form at your next appointment.
- You can download a PDF version of the form by clicking here and then emailing the form to reception@waratahgeneralpractice.com.au
- You can complete the registration form via AutoMed by clicking here or scanning the QR code.
Once your form is completed via AutoMed, it will be automatically sent to us.

Please note we are not currently able to take new patients until February 2026.
Due to roadworks in surrounding streets commencing on Monday 13th October 2025, please ensure you leave plenty of time for parking prior to your appointment time.
Ramp access at the rear of the building. No patient parking out the back. One disabled parking spot only available.

Open Wednesday 24th December 2025 8.30am – 1.00pm
Closed from 1 pm Wednesday 24th December. Reopening Monday 5th January 2026 8.30am
Online repeat scripts/referrals will also be unavailable whilst we are closed.
Please check you have enough medication for the period we are closed.

Waratah General Practice is a private billing practice.
New patients please fill out the “New Patient Information Form” found below, prior to visiting.

Waratah General Practice participates in the GP Access after-hours service.
For assistance after hours please phone (02) 4926 0500.
In an emergency dial 000, or go immediately to John Hunter or Mater Hospital Emergency Department.

An onsite Pathology collection service is available
Monday to Friday from 8:30 am to 4:00 pm
